Diane Lasichak is a professional Certified Aging Life Care Manager (formally known as a Geriatric Care Manager) with over 40 years of in-depth experience working with seniors and their families in independent living, assisted living, skilled nursing and mental health facilities. Through her education and this work experience, Diane has recognized expertise in several fields related to care management, including gerontology, social work and psychology, with a specialized focus on issues related to aging and elder care.
Through AACT, Diane provides a variety of services for seniors and caregivers contemplating or making care transitions. This process starts with an intake interview, followed by an in-depth in-person assessment that resulting in a detailed care plan.
In this process, Diane helps individuals and/or their families assess seniors’ physical, psycho-social and functional status, identify possible short-term and longer-term healthcare needs, and then help them know and understand senior healthcare options and recommendations that can be formulated into workable, informed and easy-to-understand healthcare plans.
There are up to four steps of professional Certified Care Manager (CMC) services:
Step 1: Conduct an In-person Interview and Initial Intake Assessment;
Step 2: Conduct an In-depth Assessment and Make a Care Plan;
Step 3: Identify Needed Services and Possible Service Providers;
Step 4: Monitor Care Plan Implementation and/or Changes in Needs.
Diane is a member of the National Academy of Certified Care Managers and the Aging Life Care Association of Professional Aging Life Care Managers (ALCA) and adheres to the ALCA Code of Ethics and Standards of Practice. Diane is also a member of the National Association of Social Workers (NASW).
AACT also provides consultant services in designing, planning, improving, coordinating services, and evaluating wellness and healthcare programs focused primarily on age 55+ adults, as well as training for staff on working with seniors and their family members.
